In order to perform a market study we would require the following:

• An Agent of record (AOR) or Agent of Authority (AOA)
  
  
• Last 2 years of claims experience
  
  
• Employee Data (Data Sheet)
  
  
• Questionnaire (Questionnaire); This can be completed by a Benefits Consultant with you.
  
  
• A copy of your current benefits booklet or schedule of benefits.
