Plan Enrollment

It is recommended you contact us before filling out the enrollment forms.

Enrollment is a quick and simple process. Fill out a Company Application for your business and for each employee have them fill out an Employee Enrollment Form. Once you have completed these send them to our Halifax office:

Once you have mailed the applications we will contact you if we need to obtain any more necessary information or forms.

The applications can be difficult to fill out at times so please do not hesitate to contact us should you need any assistance.
